These Terms & Conditions ("Terms") govern your access to and use of the Health Nourish True website and our home health & wellness management services. By using our website or services, you agree to these Terms. If you do not agree, please do not use our services.

1. Who We Are

Health Nourish True, founded in 2022 and headquartered at 1500 E John St, Knox, Indiana 46534, USA, coordinates health and wellness services — including preventive care, nutrition, dental referrals, cardio monitoring and related offerings — through licensed U.S. professionals and vetted partners. References to "we", "us" and "our" include our employees, clinicians and authorized contractors.

2. Eligibility & Accounts

• You must be at least 18 years old to purchase a membership. Minors may receive services only under a parent or guardian's membership and consent.
• You agree to provide accurate, current information and to keep it updated.
• You are responsible for safeguarding your member-portal credentials and all activity under your account.

3. Nature of Services — Medical Disclaimer

a. Wellness coordination, not emergency care

Our services support wellness coordination, prevention and lifestyle management. We are not an emergency service. If you experience a medical emergency, call 911 or go to your nearest emergency room immediately.

b. No guarantee of outcomes

Content on our website — including articles, plans and statistics — is for general informational purposes and is not a substitute for professional medical advice, diagnosis or treatment. Individual results vary; we do not guarantee specific health outcomes.

c. Clinician relationships

Clinical decisions are made solely by licensed clinicians exercising independent professional judgment. A clinician-patient relationship is formed only through our formal intake and engagement process, not by browsing this website or submitting a form.

4. Memberships, Billing & Renewal

• Membership plans (Essential, Complete Care, Family Elite) are billed monthly or quarterly in advance, in U.S. dollars.
• Subscriptions auto-renew until cancelled. You may cancel anytime from your portal or by contacting us; cancellation takes effect at the end of the current billing period.
• Quarterly pricing reflects a discount for prepaid quarters and is non-pro-rated mid-quarter upon cancellation, except where required by law.
• New members are covered by a 14-day money-back guarantee from first payment.
• Failed payments may pause services after notice; returned payments may incur a $25 processing fee.

5. Appointments, Cancellations & In-Home Visits

• Please give at least 24 hours' notice to cancel or reschedule visits; late cancellations may incur a fee at our discretion.
• For in-home visits, you agree to provide a safe, accessible environment for our staff. We may decline or end a visit if safety cannot be reasonably assured.
• Visits in 40+ supported metro areas are subject to clinician availability; telehealth is available in all 50 states, subject to state licensure rules.

6. Telehealth Consent

Where you use telehealth services, you consent to receiving care via secure audio-video or messaging technology, understand its limitations (including technology failures and the inability to perform physical examinations), and may withdraw that consent at any time without affecting your right to future in-person care.

7. Acceptable Use

You agree not to: misuse the website or portal; interfere with security features; scrape, resell or commercially exploit content; impersonate others; submit false health information; or use our services for any unlawful purpose. We may suspend accounts for violations.

8. Intellectual Property

All website content, branding, care frameworks and materials are the property of Health Nourish True or our licensors and are protected by U.S. and international law. You receive a limited, revocable, non-exclusive license to access the site for personal, non-commercial use.

9. Third-Party Providers

Services delivered by partner dentists, laboratories, specialists and pharmacies are governed by those providers' own terms and clinical standards. While we vet partners carefully, we act as a coordinator and do not guarantee third-party outcomes or availability.

10. Limitation of Liability

To the maximum extent permitted by law: (a) our services are provided "as is" without warranties of any kind beyond those stated herein; and (b) our aggregate liability arising from or relating to the services or website is limited to the amount you paid us in the twelve (12) months preceding the claim. Nothing in these Terms limits liability that cannot be limited by law, including liability arising from the professional negligence of licensed clinicians, which is governed by applicable healthcare law.

11. Indemnification

You agree to indemnify and hold harmless Health Nourish True, its clinicians, employees and partners from claims, damages and expenses arising from your violation of these Terms, your misuse of services, or information you provide that is false or misleading.

12. Dispute Resolution & Governing Law

• These Terms are governed by the laws of the State of Indiana, without regard to conflict-of-law principles.
• The parties will first attempt good-faith resolution for 30 days. Unresolved disputes will be settled by binding arbitration in Starke County, Indiana, under AAA rules, except that either party may bring claims in small-claims court.
• Claims must be brought individually; class actions are waived to the extent permitted by law.

13. Changes to These Terms

We may modify these Terms; the current version will always be posted here with a revised date. Continued use after changes take effect constitutes acceptance. Material changes affecting active memberships will be communicated in advance.

14. Severability & Entire Agreement

If any provision is held unenforceable, it will be modified to the minimum extent necessary and the remainder will stay in force. These Terms, together with our Privacy Policy and any member agreement, form the entire agreement between you and Health Nourish True.
